Affiliate Membership

  • Any other person who has or has had
    direct relations with the university

  • Can be a scanned signature with white background or a digital copy of signature

PROOF OF PAYMENT

  • If graduated within the last 6 years – screenshot of student ledger (to determine amount deducted for Alumni Fee)
  • If graduated more than 6 years – screenshot of payment receipt (Amount: Php1,750)
